And still, at 2am, the same question surfaces: what is actually wrong with me.

Here's what rarely gets said out loud in a psychiatric office: mental health is almost never explained by one factor. A mood that won't lift, a mind that won't quiet, a body that feels like it's bracing for something that already happened — these are usually the downstream effect of several things happening at once. A nervous system still living in an old threat response. Hormonal shifts reshaping mood and cognition. Experience the body never got to finish processing. Sleep, nutrition, and daily rhythm patterns no one ever asked about. A life structured around surviving rather than living.

Treating one piece of that in isolation can help for a while. It rarely holds.

Understanding tends to come before healing, not after it — not analysis for its own sake, but the kind of understanding that finally lets a person make sense of their own reactions, their own exhaustion, their own history. R4 exists to listen to that fully, before deciding what to do about it.

I built R4 because I asked the same question you're asking.

I'm Vanessa — a board-certified psychiatric nurse practitioner, a Marine Corps veteran, and for years, a patient inside the very system I was trained to work within.

My path into this field started with my mother's cancer. I became a nurse because I wanted care that meant something, not care that simply moved people through a system. Watching her go through oncology taught me early how much gets lost when treatment forgets the whole person.

Across more than a decade working in crisis care, geriatric care, child and adolescent care, and outpatient behavioral health — while also navigating my own experiences within the healthcare system — I kept noticing the same question: why were so many people surviving, yet so few truly recovering? Medications often reduced symptoms. Therapy created insight. But many people still struggled to reconnect with their health, purpose, relationships, and themselves.

My own search for answers extended well beyond the exam room. Through nervous system regulation, breathwork, lifestyle medicine, community, and personal healing, I experienced firsthand that meaningful recovery is rarely the result of a single treatment. Years later, I encountered ketamine-assisted therapy through my own clinical training — and what it gave me went far beyond a new tool to offer clients. Personally, it became an unexpected layer of healing: for trauma I hadn't fully processed, and for a nervous system that had been carrying more than I realized. Used thoughtfully and alongside comprehensive care, it affirmed everything I already believed rather than changing it — not because it was a cure, but because it could help create the conditions for deeper healing to take place.

Those experiences — professional and deeply personal — became the foundation for the R4 Method: an approach designed to move beyond symptom management and toward restoring the whole person.

A structured intake and case formulation — a full history across psychiatric, relational, somatic, and spiritual patterns — completed before any treatment decision is made.

What this feels like

Telling your story to someone actually listening. Not to be diagnosed on the spot, not to be rushed — to finally lay it down. Vanessa prays over each client's process before it begins, including the people who haven't found their way to R4 yet.

What changes

Clients often describe feeling lighter simply from being fully heard, before treatment has even begun. The decision to show up is often the first real shift.

02 Regulate

Helping an overworked nervous system remember what safety feels like

Clinical explanation

Nervous system regulation work — breathwork, somatic practice, heart rate variability training — designed to shift a client out of chronic sympathetic activation before deeper processing begins.

What this feels like

Your body has been bracing for a long time. This phase isn't about pushing through anything. It's about your system finally being given permission to stand down.

What changes

A grounded, calmer state many clients haven't felt in years — often the first tangible, felt evidence that something is different.

Commonly used

Breathwork, somatic exercises, heart rate variability training, guided downregulation practices.

03 Reset

The medicine session itself. A window, not a destination.

Clinical explanation

Ketamine-assisted therapy, administered in a monitored clinical setting with a licensed psychiatric nurse practitioner present throughout. Ketamine creates a temporary window of increased neuroplasticity — old, deeply worn mental patterns loosen enough for something new to take hold.

What this feels like

Think of fresh snow falling over old, worn paths through the woods. For a window of time, new paths become easier to walk. The medicine doesn't do the walking for you — it just softens the ground.

What changes

Many clients describe accessing insight, self-compassion, or emotional release that talk therapy alone hadn't reached. This varies from person to person — R4 doesn't promise a dramatic experience every time.

Commonly used

Ketamine-assisted therapy, in-person, clinically monitored; present-but-unintrusive therapeutic support throughout the session.

04 Rebuild

Turning insight into a different way of living.

Clinical explanation

Structured integration — translating what surfaced during the medicine session into concrete, trackable behavioral change, rather than letting insight fade with time.

What this feels like

This is the part that actually lasts. Insight by itself doesn't change a life. What you do with it, repeated over weeks, does.

What changes

Behavior becomes habit, habit becomes identity. This is typically where clients describe the shift from surviving to living as something felt in ordinary days, not just in session.

Commonly used

Structured integration sessions, the R4 Living Workbook, breathwork and nervous system resources, one concrete action identified per session — not ten. As R4 grows, this phase is expanding to include nutrition, peptide therapy, hormone optimization, and other body-based support suited to each client's biology.

At R4, our hope is that what begins as treatment becomes understanding, what becomes understanding grows into confidence, and what becomes confidence ultimately leads to independence.

A 90-minute psychiatric evaluation and medical assessment — in person or via telehealth — that confirms whether ketamine-assisted therapy is appropriate for you, and begins building your individual R4 plan. This is not a sales consultation. It's the beginning of understanding.

Who it's for

Adults who've already pursued other treatment for depression, anxiety, PTSD, mood disorders, or substance use concerns — including medication and/or therapy — without full relief. Also appropriate for significant trauma histories, including without a formal PTSD diagnosis.

Who it isn't for

Anyone seeking only a psychedelic experience; anyone for whom this would be a first-line treatment without having pursued other psychiatric or behavioral health care; anyone currently manic or psychotic.

Every phase of R4 is measured, not assumed. Clients complete validated clinical screening at every visit, ongoing safety checks throughout, and structured outcome tracking at key milestones — not just an intake form filed away and forgotten.

Ketamine-assisted therapy is administered under direct clinical oversight in every session, with a licensed psychiatric nurse practitioner present throughout. This is not an unsupervised experience. It's a clinically monitored medical process, held inside a rigorous and deeply human standard of care.

Questions

What you're probably wondering.

Will you take me off my medication?

Not automatically, and not as a goal of your first visit. Medication is never viewed in isolation at R4 — it's regularly reassessed as one part of a broader plan, and any change is made thoughtfully and collaboratively, based on what's actually supporting your stability and progress.

Do you prescribe medication?

Yes, when it's clinically appropriate. As a board-certified psychiatric nurse practitioner, Vanessa provides full medication management — but R4 isn't a traditional medication management practice. Medication is used thoughtfully, alongside psychotherapy, ketamine-assisted therapy, nervous system regulation, and lifestyle work, not in place of it.

Is ketamine required to become a client at R4?

No. Ketamine is one of many evidence-based tools available within the R4 Method — not the right choice for everyone, and never required.

Every client begins with a comprehensive evaluation to understand the biological, psychological, lifestyle, and nervous system factors behind their symptoms. From there, we build an individualized plan that may include psychotherapy, nervous system regulation, lifestyle optimization, medication when clinically appropriate, ketamine-assisted therapy, or other evidence-based approaches — whatever combination actually supports long-term healing, not a default path toward ketamine.

Will insurance cover this?

R4 is intentionally an out-of-network, self-pay practice. Working outside insurance requirements lets treatment be shaped by clinical judgment — the time, evaluation, and combination of modalities each person actually needs — rather than by what a plan will reimburse.

Payment is due at the time of service. On request, we provide a detailed superbill you can submit to your insurance company for possible out-of-network reimbursement. Coverage and reimbursement rates vary by plan, so we encourage you to contact your carrier beforehand to ask about your out-of-network mental health benefits, deductible, and documentation requirements. We can't guarantee reimbursement, but we're glad to support you in pursuing it.

How do I know if I'm a candidate?

The Foundations Session is how that question gets answered — through a real clinical evaluation, not a guess based on a form.

What if therapy hasn't worked for me?

That's who R4 was built for. Most clients arrive having already done real work in therapy and medication. R4 doesn't replace that work — it addresses what talk therapy and medication alone often can't reach: a nervous system still carrying what the mind has already tried to think its way through.

Yes — R4 offers a standing discount across all current and future offerings for veterans and first responders, plus a dedicated reduced rate on R4 Release. Details are shared during your Foundations Session.
